My beloved patients,
People are important and change is inevitable. I am writing to inform you after 40 years of private practice, I am planning to retire September 30, 2022. It has been an honor and a privilege to care for you. I have been blessed to live out my childhood dream of being a physician.
My plan is to spend more time with family, especially my wife who has been retired for several years, my father who is approaching 99 years of age, as well as my grandkids. I also plan to continue my civic engagement, trying to make a positive difference in the lives of others.
I am leaving you in the care of some extremely competent, professional, and talented physicians and staff at Carlos and Parnell MD PA. They will continue to provide you with outstanding care.
Your medical records are confidential and will remain on file at Carlos and Parnell MD PA. If you choose another physician within this office, you do not need to take any action concerning your records. If you choose a physician outside of this office, you will need to sign an authorization form before we can release your records. The authorization form can be downloaded from our website, www.carlosparnellmd.com or obtain the necessary form from your new physician.
