This page provides information about using our web site and who to contact for problems. Please contact us at 972-566-4862, if you have a question that is not addressed below.

 

Please, try the “I forgot my password.” link. It will lead you to answer your security questions, and then it will email you a temporary password that you should copy and paste exactly into the login page.  If you cannot get this to work or if you do not remember the answers to your security questions please call us at 972-566-4862.
Please click on the “Online Patient Services” link in the top bar and then click on the “Contact Your Physician” link. Then you will simply choose your provider, type your message and send. Please remember that certain Internet browsers limit you on the amount of characters you can type in the message box. If you are receiving an error message after you try to send, try and shorten your message and send again.
Under the “My Medical Summary” tab you will have a box called Labs, here you can click on the individual labs to see results and to print. If you do not understand your results please remember that your doctor has sent or will be sending you a secure message to tell you if they are normal or abnormal, if abnormal he will also tell you what the next step is.
If you go under the Online Patient Services, there is a link called Update Pharmacy and there you can update your pharmacy.
If you click our Online Patient Services at the top, there is a link called Update Emergency Contact and there you can update your emergency contact details.
Please give us 1-2 business days to reply to your messages. If you need a same day appointment please call the office at 972-566-4862. If you are having an emergency, please call 911 immediately; do not send a secure message to your physician!
Our providers are contracted with most major health plans and submit to most major insurances. Because plan memberships are constantly changing and renaming, we encourage you to verify participation of our providers with member services at your insurance company or through your human resources department at your place of employment to maximize your insurance benefits. Please understand that there may be charges which are not covered by your insurance plan. You will be responsible for these charges. We contract with Lab Corp, Quest, and Propath to provide our laboratory services. You will receive an invoice from them for any outstanding charges.
Carlos & Parnell, M.D. accept cash, checks, and most major credit cards: Visa, Mastercard, Discover, and American Express. Payment for services is expected at the time of visit; with verification of plan participation, you may be required to make a co-payment. A schedule of our fees can be provided for you; we would be happy to inform you of estimated fees when scheduling an appointment.
